LONG TERM CONDITION REVIEWS
BIRTH OF MONTH RECALL SYSTEM
The practice pro-actively recalls patients for an annual review of their long term condition using a birth month recall system to ensure our patients are provided with the best care possible.
The following process will be followed for your annual review:
- You will be invited for your annual review just prior to the start of your birth month. This will be sent as a letter, text or phone call
- Some appointments can be booked using a booking link which will be sent to you via text message
- Others appointments will be arranged via our coordinator. If you are unable to attend suggested appointments you will have the opportunity to contact our coordinator to discuss alternative dates/times
- Your annual review will usually involve an appointment with one of our nurses or healthcare assistants to take a blood sample, blood pressure check, foot check if you are a diabetic and lifestyle advice. Depending on your condition you will also be given a follow up review with a specialist nurse or GP. We aim to provide your annual reviews in as few appointments but some reviews require input from a clinician trained in a specific disease area, such as diabetes, asthma or COPD.
