How do I order a sick note?
All patients can self certify for the first 7 days of sickness. If an illness occurs that requires you to be off work longer than this a fitness for work certificate will need to be issued by a GP, Practice Nurse, Nurse Practitioner or Physiotherapist. Please submit details to the practice via eConsultation via the link on the home page of this website. The clinician may wish to see you or discuss this further over the telephone.
If you require long term ongoing fitness for work certificates please also submit an eConsultation. Provide the surgery with details of when your last certificate has run out and a message will be passed to the GP; if the GP needs to discuss anything they will call you later that day.
