  • Travel Clinic (this is a private service and incurs a £15 fee) – this service is currently limited due to staff sickness. please enquire with reception

LONG TERM CONDITION REVIEWS

BIRTH OF MONTH RECALL SYSTEM

The practice pro-actively recalls patients for an annual review of their long term condition using a birth month recall system to ensure our patients are provided with the best care possible.

The following process will be followed for your annual review:

  1. You will be invited  for your annual review just prior to the start of your birth month. This will be sent as a letter, text or phone call
  2. Some appointments can be booked using a booking link which will be sent to you via text message
  3. Others appointments will be arranged via our coordinator.  If you are unable to attend suggested appointments you will have the opportunity to contact our coordinator to discuss alternative dates/times
  4. Your annual review will usually involve an appointment with one of our nurses or healthcare assistants to take a blood sample, blood pressure check, foot check if you are a diabetic and lifestyle advice.  Depending on your condition you will also be given a follow up review with a specialist nurse or GP.  We aim to provide your annual reviews in as few appointments but some reviews require input from a clinician trained in a specific disease area, such as diabetes, asthma or COPD.
