Chronic Kidney Disease

The practice has a very structured approach in helping and assessing patients suffering from chronic kidney disease. Julia is our nurse specialist responsible for co-ordinating the care of this group of patients.

We measure patient’s kidney function regularly if they suffer from a variety of long term conditions such as diabetes, heart disease and blood pressure. This may be by urine tests or blood tests and will help for us to identify problems early.

It is very important that patients have good lifestyle advice and, in particular, try to stop smoking, take regular exercise and lose weight where appropriate.

If a patient is diagnosed as having kidney disease then the practice will monitor the blood and urine regularly (yearly if mild or every 6 months if more severe). The practice will refer the patient to the renal team whenever appropriate such as rapid deterioration in the kidney function, or if other conditions are suspected.
